中文摘要: 叶轮式左心辅助装置(LVADs)广泛应用于终末期心衰的治疗和心肌功能的恢复,为了提高使用者的生存质量,搏动转速控制逐渐取代恒定转速控制成为近年来研究的热点。但是由于目前没有针对心血管系统的切换性开展深入研究,LVAD控制指标的选择一直缺乏理论依据。本项目首次提出利用切换系统理论研究心血管优化耦合原理的思想,进而建立能够维持左右心室做功均衡的生理控制指标。主要研究内容包括:(1)利用心肌力的切换特性构建一种新的双心耦合动力学模型;(2)利用时间尺度变换法、驻留时间法等研究左心室的搏动规律和心血管系统的切换稳定性;(3)基于左心室最小功原理和双心耦合模型,利用庞特里亚金极大值原理和智能优化算法研究转速控制规律。本项目旨在利用切换系统理论和最优控制理论为LVAD的搏动转速控制建立统一的理论框架;为研究心血管优化耦合问题提供一种新的思路;从血泵控制的角度为解决左心辅助右心衰问题提供更有效的方案。

英文摘要: Impeller left ventricular assist devices(LVADs) have been widely used in end-stage heart failure therapy and myocardial recovery. In order to improve the quality of life of LVAD recipients, pulsatile speed instead of constant has become a research focus for LVAD control in recent years. However, due to the absence of in-depth studies on switching properties of cardiovascular system, LVAD control criteria have been selected without sufficient theoretical basis. In this project, a novel idea is proposed to study ventricular-vascular optimal coupling principles by means of the switched system theory, and then a physiological control criterion for maintaining the balance between left and right ventricular work is established. The main contents include: (1) A novel dynamic model incorporating dual ventricular coupling effects is developed based on the switching property of myocardial force; (2) The time-scaling transformation and dwell time methods are used to study the pulsatile characteristics of left ventricle and the switching stability of cardiovascular system; (3) Based on the minimum work criterion of left ventricle and the dual ventricular coupling model, the optimal speed law is derived using the Pontryagin maximum principle and intelligent optimization algorithms. The aim of this project is to establish a unified framework for the pulsatile speed control of LVADs by means of switched system and optimal control theories, provide a novel approach to study the ventricular-vascular optimal coupling problem and provide a more effective scheme from the viewpoint of LVAD control to deal with the right ventricular failure during left ventricular assistance.
