中文摘要: 束流调强技术(IMRT)及容积弧形调强技术(VMAT)是目前放射治疗的主流技术。但IMRT优化方案复杂、布野依赖人工、治疗效率低;VMAT技术在保证高效放疗的同时相对简化射野各方向上强度优化,降低了放疗优化方案的质量。本研究将着重研究VMAT优化模型的两个均匀性:“①弧内子野个数均匀等分”②“等分的子野同方向上数目单一”并进行改良,构建非均匀VMAT优化模型,自主研发创建非均匀VMAT新技术。新技术在保留常规VMAT高效性的同时结合IMRT特点加强照射野方向上的优化,可以提高放疗方案的质量、有效减少正常组织照射容积、保护正常器官;此外,新技术实现所研发的人工智能放射治疗优化软件,能够减少人为原因造成的误差,提高放射治疗计划的制作精度及效率,最终改善患者的治疗疗效和生活质量。本研究探讨的新技术具有效率更高、更智能的优势,在医疗人力资源紧缺的今天,尤其具有现实意义且符合肿瘤放疗的未来发展趋势。

英文摘要: As the mainstay techniques in radiation therapy, IMRT and VMAT produce highly conformal dose distribution for most of the complex targets while sparing critical normal tissues or organs. However, the IMRT technology has been proved to be very complicated and of limited efficiency. With the improved efficiency, VMAT technique has to tolerate compromise of plan quality due to minimization of intensity modulation for fast delivery of treatment plan. . Based on our previous work, this project aims to optimize the VMAT intensity modulation to achieve better balance between plan quality and delivery efficiency. A non-uniform VMAT optimization model will be established to develop a specific non-uniform VMAT technique with the purpose of generating a single arc VMAT-based plan with the non-uniform beams and intensity modulation levels using our customized software tools. The verifications will be conducted using patient imaging database with various clinical diseases. Dosimetric parameters, quality assurance and the feasibility of the machine delivery will also be evaluated.. The proposed non-uniform VMAT technique is to design a novel, effective approach to produce a single-arc plan with integrating advantages from current IMAT and VMAT for improving plan quality and delivery efficiency.
